Waiver of Liability: I, the undersigned, wish to participate in dance classes, workshops, or activities offered by Ingram Entertainment. I understand that dance classes involve physical activity and carry certain inherent risks, including but not limited to sprains, strains, falls, and other injuries. I acknowledge and voluntarily assume these risks. In consideration of being permitted to participate in dance classes, I hereby agree to release, discharge, and hold harmless Ingram Entertainment, its instructors, employees, and agents from any and all claims, demands, causes of action, or liability for any injuries or damages sustained during or as a result of my participation in the dance classes.

Medical Information: I hereby certify that I am in good health and have no physical conditions or medical reasons that would prevent my participation in dance classes. I agree to inform [Your Dance Studio Name] of any changes to my health status.

Photography and Likeness Release: I grant [Your Dance Studio Name] the right to take photographs and/or videos during dance classes and to use my name, image, and likeness in promotional materials, including but not limited to the studio's website, social media, and advertising materials.
